PARENT AND PROVIDER CONTRACT
HOURS OF OPERATION: 7:30 am – 4:30 pm Monday through Friday. Drop in care welcome upon availability. Evenings and weekend care available on occasion.
CONTRACTED CHILDCARE: Parents are to provide a firm schedule as far in advance as possible. Care will be provided for the following days and hours. Part time contracted care is accepted. If the time arises when part time hours vary, they may be changed if a one-week notice is given, and provider’s ratios are not compromised. All care begins with the teacher’s return to work after the summer break and ending with the last week of school.
TUITION FEES: The rate for tuition is $_________per day, $_________per week, for the following child/children. Any hours over the specified time will be considered overtime. Name of your child/children___________________________. This fee is due regardless of attendance. The first two week’s tuition will be due before the child starts at the Academy. Summer tuition will be as follows; full tuition is due to hold the child's spot until school resumes. This money does not apply toward future tuition and is non-refundable should you decide not to come back after the summer break.
PAYMENT: Payment will be made on Friday in advance, no later than 6:00 pm. If the fee is not paid by 6:00 pm on Friday in full, a $10.00 a day late fee will be added to the amount due. Checks are accepted; however, if you have a check return, there will be a $25.00 fee. In addition, any fee incurred by the provider for non-payment of check will be assessed to the parent. After one returned check, only cash will be accepted. **PLEASE NOTE** If your child/children will not be here on Friday, payment is due on the last day the child is in care.
OVERTIME: Overtime is considered outside that which is agreed upon, specifically dropped off before the hour of _____am and picked up after _____pm. The following charges will be assessed time incurred payable the same week of service.
** $6.00 per hour, of prearranged overtime, per child (with a 24 hour notice) or ** $1.00 per minute, per child when it is not prearranged. The provider is under no obligation to provide an extension of time. ** Late arrival does not justify late departure**
BAD WEATHER DAYS: When the Arlington Independent School District closes due to bad weather, ABC Baby Academy will also be closed. If there is bad weather the provider will call the Parents/Guardians by 9 pm and advise what procedure will be followed the next day. The provider will inform Parents/Guardians if the academy will be opened for the day, open half a day, or closed all day. These days are paid days.
DROP OFF /PICK UP: Parents must drop off and pick up their child/children in a timely manner in order to prevent disruption of the childcare facility. Children should not arrive early or stay late unless prior arrangement has been made. Parents mush furnish in writing the names of all persons who are authorized to pick up their child/children. A photo ID of each person authorized must be furnished and on file. In case of emergency, please telephone me ahead of time and give me the name of the person you have authorized to pick up your child/children. I will require ID before releasing your child to anyone who has been previously approved.
OPEN DOOR POLICY: Parents/Guardians are welcomed to visit unannounced. The Parents/Guardians personal visit can last no longer than 30 minutes. If Parents/Guardians wish to stay longer than 30 minutes, a criminal FBI background check must be completed and on file with the academy.
CHILDCARE HOLIDAYS: The Academy observes Arlington ISD school calendar Holidays including Christmas, Spring break and Teacher in-service days. Parents are responsible for finding back up care during professional and emergency closings. September two State training days. Five sick days. Five personal days.
PEANUT’S OR PEANUT PRODUCTS: Peanuts and/or peanut products are used at this childcare facility. Allergies to this product in young children may not be known yet. Allergies to this product may present it self while at childcare. If an allergic reaction does happen, all emergency procedures will be taken to assist your child. Please report all know allergies.
ILLNESS: For the health of all the children I care for, please do not bring your child to the academy if he or she has an illness i.e., cold, flu are any mucous drainage. If your child becomes sick in the childcare facility, you’re responsible for picking up him or her in a timely manner. If your child reaches 100.4 (by mouth and 99.4 under arm) degrees or develops any illness as described above, I will contact you regarding pick up. Please do not bring your child back, until he or she has been free of fever and all symptoms of the illness for at least 24 hours without the aide of medication.
LEAVE OF ABSENCE: In the event you want to take a leave of absence, a two-week written notice is required; along with two weeks full pay when the leave starts. One half pay is required thereafter. This holds your child’s spot. A leave of absence will last no longer than four weeks. After the fourth week, full pay is expected or the spot will be considered open and available.
ANIMALS/ OUR BABy: I would like to introduce our baby Mozart Bentley a white Bichon Frise this breed of dog does not shed. In the 1800’s these dogs were lap dogs and very cherished, they were sometimes used for bartering when needed. He is a very happy baby who just wants to love everyone and go bye-bye with anyone.
MEDICATIONS: If your child needs prescription medicine administered, please sign the “Medicine List” stating the times and the amount of medicine to be given to your child. All prescribed medicine provided must be clearly labeled with the child’s full name and directions for the proper amount (dosage) and time to administer. The only non-prescription medicine that will be administered at ABC Baby Academy will be teething tablets and emergency only Tylenol (will be given when the fever rises to 101).
EMERGENCY MEDICAL CARE: If a child is seriously injured or becomes seriously ill, I will administer CPR and/or appropriate First Aid until EMT’s arrive, I will call 911 immediately for assistance. I will contact the parents as soon as possible with information deemed from the paramedics.
IMMUNIZATIONS: Immunizations are a state requirement. If your child/children does not have current immunizations, it is a cause for immediate termination or exclusion from the academy. If he or she is not current, a doctor’s note or a written religious objection is required. This facility requires a copy of your child’s shot record.
RECORDS/FORMS: All records and forms are legally binding documents, which must be brought on or before the child’s first day. Parents/Guardians will make sure the meaning is understood before signing.
CONTRACT AMENDMENTS: The Provider, Bobbie DeLany reserves the right to make changes to this contract at anytime. A one-week notice will be given to parents if a new contract will be presented for signatures. The new contract will be as binding as the previous contract.
TERMINATION OF SERVICE: Giving a two-week written notice may terminate this contract for care. If the parent does not provide a two-week notice, fee for the two weeks is still required. I retain the right to terminate this contract WITHOUT NOTICE in the event of destructive, uncontrollable, violent behavior, or delinquent fee. The following is considered unacceptable behavior and if it persists, will be grounds for termination: biting, hitting, scratching, pinching, throwing objects, or rough behavior that is harmful to others.

DISCLAIMER: I/We herby agree to comply with the rules and regulations of ABC Baby Academy, regarding fees, holidays, health and other specified. I/We are aware of the scheduled childcare holidays (with pay). I/We will be notified in writing of any contractual changes, before they go into effect. By signing this agreement, I/We agree to honor all terms of this contract, and the written policies of ABC Baby Academy, now and in the future. Breach of contract in any way by the parent may result in immediate termination of all services.
I have been made aware that ABC Baby Academy does use powder, lotion, oil and diaper rash ointment as needed.
